This is an In-Office Membership, NOT dental insurance. It cannot be used in conjunction with the following:

Discounts cannot be applied to treatment by a specialist at any other dental office. Annual Plan fee is non-refundable and non-transferable.

Payment for treatment is due at the time services are rendered.

The Wangsgard Family Dental In-Office Membership Plan is valid for one year of service from the date of initial payment. All patient portions must be paid at the time of service. Renewal rates may be subject to revision annually.
